If you are a hospital, healthcare provider, or authorized representative requesting medical records for a patient, please submit your request using the secure form below. You may also fax requests to our office at 915-209-7566 or email us at DaniellaRojas@familymhmrcenteroftexas.com with a signed release of information. For the privacy and safety of our patients, all requests must include the proper authorization form and patient identification—requests missing the required documentation cannot be processed.
